YourHealth Essential, a free application allows you to control, consolidate and manage your critical health information from disparate health disciplines so you are prepared before a medical issue or a trip to the emergency room. This includes contacts, medical conditions, medications, allergies, vaccinations even your supplements and over the counter medications. When you enter this information, YourHealth Essential will automatically create your personal Essential Care Card. With your contacts, you can provide critical care needs to EMTs, doctors, caregivers and neighbors; this is your YourHealth Care Circle™ of trusted people to care for you in an emergency.
YourHealth Essential makes it easy to provide this information through personalized QR codes. You can keep these QR codes on your phone, in your wallet or any place you choose. In addition, you can send the QR code via email or SMS to people you have selected.
